Job DescriptionThis Clinical Case Manager position is 100% remote and candidates can apply from any state but must be able to work EST hours. The Clinical Case manager is a licensed Clinical Social Worker position. Normal Working Hours: Monday through Friday 8:00am-5:00pm EST with flexibility to work outside of these standard hours per the business need.No evenings, weekends, holidays, or travel will be required. The Clinical Case Manager partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs.The Clinical Case Manager will work with a population spans the entire lifecycle. All member contact is done telephonically or via electronic exchange. The Clinical Case Manager's role is to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.Pay RangeThe typical pay range for this role is:Minimum: 45,240Maximum: 98,280Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications-Candidate is required to have one of the following licenses: Licensed Masters Level Social Worker (LMSW/LSW), Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker (LISW).-Must have an unrestricted active license-Must be willing and able to obtain multi-state licensure (minimum of 10 states) before new hire training is completed (additional licensures will be paid for by the company).-2+ years' experience in medical social work or case managementPreferred Qualifications-Managed Care experience-Telephonic Social Worker experienceEducationMaster's Degree Licensed Social WorkerBusiness OverviewBring your heart to CVS Health Every one of us at CVS Health shares a single, clear purpose: Bringing our heart to every moment of your health.
