Care Management Associate

Job DescriptionSupport comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services. Case management associates (CMAs) support the CM team by performing non-clinical, administrative duties. These duties may include but are not limited to processing referrals, making outreach calls, administering Health Risk Questionnaires (HRQ's), providing appointment reminders and post appointment follow-up, and entering data into the CM business application system.Pay RangeThe typical pay range for this role is:Minimum: 18.50Maximum: 33.65Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications2-4 years' experience as a medical assistant, office assistant. Strong customer service skills to coordinate service delivery including attention to customers, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members.
