Discover more about us at This role is eligible for a $1,500 sign-on bonusThe Care Connector is responsible for supporting the daily operations of the Bright Start maternity program. The Care Connector performs in a contact center environment, effectively processing calls from Members, Providers and other areas, internal and external to the company. The Care Connector telephonically outreaches to pregnant members and assists them with scheduling appointments, identifying gaps in care/HEDIS related health conditions, and connects members to various community resources. Under the direction of Clinical staff, the Care Connector assists members in accessing care throughout the post-partum period, increasing health care adherence, and reducing barriers to care.Education/Experience:High school diploma or GED required.Minimum of 2 years previous work experience, preferably in a health care setting.Managed care and medical terminology experience preferred.Prior member service or customer service telephone experience.Excellent interpersonal and verbal communication skills with the ability to maintain confidentiality.Attention to detail with strong organizational skills.Proficient PC skills in a Windows based environment including Outlook, Word, and Excel.
