Licensed Practical Nurse

*Job Summary*We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients in a nursing homes. The ideal candidate will possess strong clinical skills and a commitment to patient-centered care, ensuring the highest standards of health and safety.*Duties** Administer medications and treatments as prescribed by physicians.* Monitor patient vital signs and report any changes to the healthcare team.* Assist with procedures such as IV insertion and botulinum toxin treatment.* Provide care for patients requiring pain management and discharge planning.* Collaborate with healthcare professionals in a Long Term settings.** Maintain accurate patient records and documentation in compliance with healthcare regulations.* Educate patients and families on health management and community resources.*Qualifications** Valid LPN license in the state of practice.* Experience in nursing home with as least 1 year preferred.* Knowledge of pain management strategies and discharge planning processes.* Strong communication skills, both verbal and written, with a focus on patient education.* Ability to work collaboratively within a multidisciplinary team.Join us in making a difference in the lives of our patients by providing exceptional nursing care.
