LPN Assessment Coordinator (LPNAC)

Transitions Healthcare Gettysburg is currently hiring a LPN Assessment Coordinator. Primary purpose in this position is to work with the MDS Coordinator to plan and implement the MDS completion in accordance with facility procedures as well as State and Federal regulations.LPN Assessment Coordinator (LPNAC) Position Highlights Include:Gathers necessary data for completion of the MDS.Assists in identifying significant changes in residents' condition.Monitors resident care plans for accuracy and completion.Attends the Medicare meeting.Assists in education of the MDS process with nursing staff and the Interdisciplinary care team.Data entry of MDS information.Reviews messages from contracted pharmacy regarding medications needing prior authorization.LPN Assessment Coordinator (LPNAC) Minimum QualificationsCurrent licensure as either a practical nurse or a registered nurse in the Commonwealth of PennsylvaniaCPR/AED certificationMinimum of one (1) year nursing experienceProficient in MS Office Suite and OutlookEoe#THCGB
