#hpsdhhLicensed Vocational NursePOSITION SUMMARY:You will be responsible for providing nursing care to assigned patients under the direct supervision of a registered nurse (RN). General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.RESPONSIBILITIES:Provide direct patient home care visits, observe, perform and report preventative, treatment, and rehabilitative nursing procedures while implementing the plan of careObserve and report on the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effectsWork with patient, family, and health care team members to coordinate services, promote quality of care and positive patient care outcomesTake verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriatelyInform Health Care Providers of changes in patient's condition in accordance with provider orders and document communications accordinglyProvide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventionsProvide instructions to patient/CG's and staff on timeQUALIFICATIONS:Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse, and CPR certificationAt least one year of clinical practical nursing experience. *Home Health experience preferredMust have a vehicle, a valid current driver's license, and automobile insuranceAbility to make home visits and provide skilled nursing assessments and care in the homeBENEFITS (available to full-time employees):Excellent pay, including 23 days of PTO per yearHealth, dental, vision, 401k, life insurance, short-term and long-term disabilityContinuing education & tuition assistanceGenerous referral bonus programFlexible schedulingCompany Vehicle Program with a paid gas card for eligible full-time employeesABOUT US:Our unique focus is on patient-centered care and evidence-based medicine.
