Hospice Night Nurse - On Call $5k Sign on Bonus

This position is responsible for completing visits that have been triages and then assigned to them throughout the night. This position is 100% remote as the RN will work from home when not out in the field providing patient care. The hours for this position are Monday - Thursday 5pm - 8:30am. This is a fulltime position and is eligible for PTO, paid holidays and benefits.*Duties and Responsibilities:** Identifying and addressing comfort care needs. Supporting, instructing and educating the patient, family and caregiver* Ensure all proper nursing documentation is completed.* Maintaining the dignity of the dying patient, supporting the patient's and family's unique spiritual and cultural beliefs* Providing holistic, family-centered care across treatment settings to improve the quality of life, and consulting and collaborating with the interdisciplinary team and others involved in the patient's care.* Initiating communications with attending physicians, other hospice team members and agencies as needed to coordinate optimal care and use of resources for the patient/family* Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status and communicate that data in team meetings to formulate an Interdisciplinary Plan of Care* Providing appropriate support at the time of death and period of bereavement* Participating in orientation and in-service training programs for Zusman Hospice staff.* Working effectively in an interdisciplinary team environment* Demonstrating a positive attitude and working relationship with staff members and others* Consistently represents Zusman Hospice to all internal and external customers with integrity and professionalism.* Provide on Call Coverage 3-4 nights per week including every other weekend.*Job Requirements** Registered nurse with current licensure with the State Board of Nursing in Ohio* Ohio driver's license, current auto insurance, and access to a car during working hours.* Hospice experience preferred.* Wellsky experience preferred.* Demonstrated organizational skills and abilities to handle multiple priorities accurately and time to facilitate customer and program needs.* Experience in human relations, sensitivity to persons in crisis, good listening skills.
