Hennepin Healthcare has a large psychiatric program, home care and hospice, and operates a research institute, innovation center and philanthropic foundation.Hennepin Healthcare (HHS) has a 0.50 FTE (40 hours per two-week pay period) opening for a MTM Clinical Ambulatory Pharmacist supporting our Ambulatory Oncology Pharmacy program by serving our Comprehensive Cancer Center and Infusion Centers. This position will work day shift, no weekends. In this role it is necessary to demonstrate sensitivity, knowledge, and understanding of diverse academic, socioeconomic, gender, cultural, and ethnic backgrounds. Our Pharmacists are credentialed and privileged through the Credentialing department at HHS. An ideal candidate would have completed a PGY-1 Residency and PGY2 Residency in Oncology with additional ambulatory oncology clinical experience thereafter. BCOP (Board Certified Oncology Pharmacist) is preferred.HHS has a long history of including ambulatory pharmacy services as part of our oncology care model. Our ambulatory pharmacists are an invaluable resource to the care team, provide direct education to patients as they navigate cancer treatment, and serve as a liaison for patients as they transition between acute and ambulatory care. This service is delivered on-site at our main campus.In addition to this role, the pharmacists will also have the opportunity to staff within our Specialty Pharmacy, additional clinics within our ambulatory system, and/or inpatient pharmacy team, depending on interest and area of training.RESPONSIBILITIESProvide Medication Therapy Management and/or disease state management to clinic patientsCritically evaluate a patient's medication profile to ensure that each medication is indicated, effective, safe, and that the patient is adherent with therapyCompletes timely documentation of activities within the electronic medical record as appropriate for specific areas of responsibilityResponsible for maintaining accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy and interaction potential of the prescribed medicationProvide medication information to patients, nurses, physicians or other health care practitionersBe able to critically evaluate and interpret medical information when providing specific information to physicians or providersPerforms all responsibilities/duties required by the pharmacy as defined by the scope of service particular to each clinic to ensure that the unique nature of the client is addressedMay precept Resident or PharmD student rotationsMaintain current knowledge of pharmacotherapy, pathophysiology, and procedures in area of practiceDuring patient encounter, responsible for interpretation of provider medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepanciesDuring patient encounter, based on an assessment of the patient's specific medical needs, making interventions with the medical staff and nursing staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy servicesKeeps up to date with competencies of both the pharmacy and those required in the clinic of practiceCompletes timely documentation of activities within the electronic medical record as appropriate for specific area of responsibility per policy from the Office of Medical Staff. Documentation is to be completed within 5 business days of encounter and no later than 30 days following the encounterMay administer vaccinations, as applicableMay work under Collaborative Practice and protocols as authorized by providersQUALIFICATIONSMinimum Education/ Work Experience:Graduation from college with a degree in PharmacyPreferred Qualifications:Currently practicing as an MTM provider in the state of MN -or-Completion of a residency program with emphasis on ambulatory care within the past two years -or-BPS certificationBCOP (Board Certified Oncology Pharmacist)Knowledge/ Skills/ Abilities:Excellent problem-solving & communication skills (verbal & written)Must be a self-starter, able to work under pressure and be flexible in setting prioritiesAbility to do accurate, detailed analysis of complex information and issues is essentialExtremely strong customer service orientationEffectively interacts with team members and other departmentsAdaptable to changing priorities, tasks, and work schedules to meet customer service standardsLicense/Certifications:Possession of a valid license to practice pharmacy in the State of MinnesotaMTM provider certification achieved through completion of a DHS approved Accreditation Council of Pharmacy Education (ACPE) program that has both didactic and clinical elements within the past two years of employmentYou've made the right choice in considering Hennepin Healthcare for your employment.
