These field-based roles cover Manhattan, Queens, and Brooklyn, with a car and valid driver's license required for Queens and Brooklyn regions.In this role, you will be the cornerstone of your patients' home care journey, taking primary responsibility for coordinating, implementing, and continuously evaluating their care needs.
