The center is committed to providing individualized, holistic care through warm and meaningful interactions, aiming to help residents achieve their best outcomes and ensure their comfort is a top priority.Location: 76 Fenton Street, Livermore 94550Website: are seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our skilled nursing facility team.RESPONSIBILITIES:Conduct comprehensive assessments to evaluate patients' speech and language abilities.Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.Provide therapeutic interventions, including but not limited to, articulation therapy, language development therapy, voice therapy, and cognitive training.Monitor and document patients' progress and make adjustments to treatment plans, as necessary.Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.QUALIFICATIONS:New passer/graduates, and CF supervision are welcomePrevious experience as Speech Language Pathologist (SLP) or Speech Therapist (ST) is a plusWith Skilled Nursing or Long-Term Care experience is a plusValid state licensure as a Speech Language PathologistActive CPR CertificationCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)Is in good standing with all regulatory agencies and licensing boards is requiredInterest in working with the geriatric populationDegree in Speech-Language Pathology from an accredited program.Strong knowledge of speech and language disorders, evaluation techniques, and evidence-based therapeutic interventions.Excellent communication and interpersonal skills.Proficiency in computer skills and electronic medical records systems.Strong organizational and time management skills.Compassionate and patient-centered approach to care.Ability to adapt to changing priorities and thrive in a fast-paced environment.
