RN Case Manager - Weekend Baylor

Job Description:ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES• Ability to work on call night and weekends as needed.• Work closely with the sales, clinical, and leadership team.• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.• Implement/develop/document the patient plan of care.• Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).• Assure continuity of quality patient care delivered with appropriate documentation.• Monitor assigned cases to ensure compliance with requirements of third-party payer.• Demonstrate commitment, professional growth, and competency.• Promote agency philosophy and administrative policies.• Perform on-call responsibilities and provide on-call service to patients/families as assigned.• Supervise and provide direction to the home health aide and the LPN.• Perform other duties as assigned by management to support department goals and initiatives.KNOWLEDGE, SKILLS, AND ABILITIES• Ability to write, speak, understand, and read the English language.• Demonstrate knowledge of and supports Affinity Hospice's vision, value statements, standards, policies, and procedures.• Excellent interpersonal, communication, and time management skills.• High organizational and work ethic standards.• Strong analytical and problem-solving skills.• Ability to work independently and as part of a team.• Must be computer literate. • Flexibility to travel within the assigned territory.• Meet mandatory continuing education requirements. • Demonstrate commitment, professional growth, and competency.Affinity Hospice is an equal opportunity employer.
