We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and provide compassionate medical care to individuals in our residential and community programs. If you are a compassionate LPN looking to make a difference, we'd love to hear from you!*Key Responsibilities:*â— Provide direct nursing care, including medication administration, wound care, and monitoring vital signs.â— Conduct health assessments and report concerns to healthcare providers.â— Assist with medication management, education, and compliance for individuals in our programs.â— Maintain accurate medical records in compliance with state and organizational policies.â— Educate and support staff and caregivers on health-related concerns and best practices.â— Collaborate with case managers, therapists, and community resources to coordinate care.â— Advocate for clients' health needs and ensure a safe, supportive environment. Qualifications:â— Active LPN License in Pennsylvania (required)â— Driver's License (required)â— Experience in mental health, intellectual disabilities, or community-based care (preferred)â— Strong communication and interpersonal skillsâ— Ability to work independently and as part of a teamâ— CPR & First Aid Certification (or willingness to obtain)â— Passion for serving vulnerable populationsJob Type: Full-timePay: $52,520.00 per year_*Minimum requirements*__: HS diploma/GED, current PA LPN license, and two (2) years of professional experience; ability to complete physical and PPD tests. Must be at least 21, possess a valid driver's license, insurable driving record, and reliable transportation. Current physical and Mantoux (TB) test required. PA State Police background check, Act 33, and/or FBI clearances.
