We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and provide compassionate medical care to individuals in our residential and community programs. If you are a compassionate LPN looking to make a difference, we'd love to hear from you!*Key Responsibilities:*● Provide direct nursing care, including medication administration, wound care, and monitoring vital signs.● Conduct health assessments and report concerns to healthcare providers.● Assist with medication management, education, and compliance for individuals in our programs.● Maintain accurate medical records in compliance with state and organizational policies.● Educate and support staff and caregivers on health-related concerns and best practices.● Collaborate with case managers, therapists, and community resources to coordinate care.● Advocate for clients' health needs and ensure a safe, supportive environment. Qualifications:● Active LPN License in Pennsylvania (required)● Driver's License (required)● Experience in mental health, intellectual disabilities, or community-based care (preferred)● Strong communication and interpersonal skills● Ability to work independently and as part of a team● CPR & First Aid Certification (or willingness to obtain)● Passion for serving vulnerable populationsJob Type: Full-timePay: $52,520.00 per year_*Minimum requirements*__: HS diploma/GED, current PA LPN license, and two (2) years of professional experience; ability to complete physical and PPD tests.
