General descriptionTo provide psycho-educational classes and individual counseling services to Kintock clients; Intake, assessment ongoing assessment for program eligibility, monitoring all aspects of care and client services, and ensuring that the needs of clients are met, may be included at discretion of Clinical Supervisor/Program Director.Job descriptionProviding alcohol and substance abuse counseling in individual and group formatsFacilitate required Cognitive Behavioral Workshops and classesConduct pre and post program assessmentsCollect and manage all pre and post assessment dataConduct crisis intervention counseling sessions with residents who either request a meeting or are referred by another staff memberDevelop a comprehensive plan of care that meets the needs of the clientProvide the client with information on community services and agency services which will address their needs and make the appropriate referrals on behalf of the client.Educate client on all areas related to program participation including rights and responsibilities and appeal rightsUtilize case management skills, such as brokering, advocacy, monitoring and discharge planning to link clients and their families to resources and servicesWork closely with client to develop and revise care plans that are client focused and meet client needsSchedule and/or participates in Case Conference with clients/caregivers, service care planning and negotiate agreementsProvide discharge planning to dis-enrolled clients to ensure clients needs are met.Maintain client physical file with required forms as per policyEnsure all services are supported by care plan, and case note documentationEnsuring that all court stipulations are met by each resident assigned to the caseloadEnsure that the duties of this position are performed in accordance with contract, ACA, licensure, and corporate standardsRequired to perform all assigned ACA related dutiesRequired to attend ACA and other assigned or mandatory training classes in accordance with Kintock training policiesQualifications/RequirementsExcellent oral and written communication skillsBi-lingual (English/Spanish) preferredMinimum six-months of relevant experience are preferredKnowledge/AbilitiesComputer skills including the Windows operating systems, MS Word, and MS ExcelLearn, Analyze and interpret understand and follow contract rules, regulations, policies, and procedures; participate as in an effective member of a program teamCommunicate effectively and professionally with staff, residents, and agency representativesEducationCADC, or a Bachelor or Master's degree in a helping profession, and eligible for licensure (or an interest in pursuing licensure as a LCADC, LAC, LPC, LSW or LCSW in the future).
