The center is committed to providing individualized, holistic care through warm and meaningful interactions, aiming to help residents achieve their best outcomes and ensure their comfort is a top priority.Location: 76 Fenton Street, Livermore 94550Website: are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility.RESPONSIBILITIES:Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.Monitor and document patient progress and adjust treatment plans as necessary.Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.Provide hands-on therapy including exercises, manual therapy, and modalities as appropriate.Ensure compliance with regulatory standards and company policies in all aspects of care delivery.Participate in clinical meetings and contribute to a culture of continuous improvement.QUALIFICATIONS:New passer/graduates are welcomeWith Skilled Nursing or Long-Term Care, experience is a plusPrevious experience as a Physical Therapists (PT) is a plusPhysical Therapy Degree from an accredited programState licensure or eligibility for licensure as a physical therapistIs in good standing with all regulatory agencies and licensing boards is requiredInterest in working with the geriatric populationStrong knowledge of anatomy, physiology, and medical principles related to physical therapyExcellent communication and interpersonal skillsAbility to work effectively both independently and within a teamStrong problem-solving and critical-thinking abilitiesProficiency with electronic medical record (EMR) systems is a plusIf you are a licensed Physical Therapist looking to make a meaningful difference in the lives of others, we invite you to apply to join our team!
