We are seeking a dedicated and compassionate Palliative Care Nurse Practitioner (NP) to join our client's interdisciplinary team, providing holistic care to patients with serious or life-limiting illnesses. The ideal candidate will have experience in palliative care or hospice, demonstrating expertise in symptom management, emotional support, and end-of-life care planning. This role involves working collaboratively with physicians, nurses, social workers, and other team members to ensure the highest quality of patient-centered care.*Key Responsibilities:** Perform comprehensive assessments of patients with chronic, advanced, or terminal illnesses, including physical, emotional, social, and spiritual needs.* Develop and implement individualized care plans focused on symptom management, quality of life, and patient goals.* Provide pain and symptom management, including prescribing medications and treatments in accordance with state regulations.* Facilitate discussions about advance care planning, goals of care, and end-of-life decisions with patients and families.* Collaborate with the interdisciplinary palliative care team to ensure a coordinated approach to patient care.* Serve as a resource and consultant to other healthcare providers regarding palliative care principles and practices.* Provide education and emotional support to patients' families and caregivers, helping them navigate the complexities of serious illness and end-of-life care.* Act as a liaison between patients, families, and the healthcare team to address concerns and ensure understanding of care plans.* Educate patients, families, and staff on palliative care principles, including symptom management and advance care planning.* Advocate for patients' needs and preferences within the healthcare system.*Requirements:** Master's degree in Nursing with certification as a Nurse Practitioner (NP).* Current NP licensure in New York.* Minimum of 1 year of experience in palliative care or hospice.* Spanish-speaking abilities preferred but not required.* Excellent communication and interpersonal skills, with the ability to have sensitive conversations about serious illness and end-of-life care.* Strong clinical judgment and critical thinking skills.* Ability to work collaboratively within a team and independently when needed.* Empathy, compassion, and a commitment to patient-centered care._Please note - The annual base salary range is $130k to $165k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
