They partner with hospitals and healthcare providers to ensure seamless transitions from hospital to home.We are seeking an experienced Homecare Field Manager to oversee patient transitions from hospital discharge to home-based care. This role involves working closely with hospital discharge planners, case managers, and healthcare providers to coordinate patient care plans, ensure compliance with homecare protocols, and support a smooth transition for patients. The ideal candidate will have a strong background in healthcare coordination, home health services, and patient advocacy.*Key Responsibilities** Collaborate with hospital discharge planners and case managers to assess patient needs and develop individualized homecare plans.* Serve as a liaison between hospitals, patients, families, and homecare providers to ensure seamless continuity of care.* Conduct patient assessments, educate families on homecare options, and address concerns regarding post-hospitalization care.* Coordinate with interdisciplinary teams, including nurses, therapists, and social workers, to arrange appropriate home health services.* Ensure compliance with healthcare regulations, company policies, and industry standards in home health coordination.* Monitor patient progress post-discharge and address any issues that may arise during the transition to home-based care.* Develop and maintain relationships with hospital staff, referral sources, and community organizations to enhance patient support services.* Provide ongoing support, training, and leadership to homecare staff as needed.*Requirements:** Bachelor's degree in Nursing* RN License Required* Minimum of 2-3 years of experience in home healthcare, case management, or hospital discharge planning.* Strong understanding of Medicare, Medicaid, and private insurance home health eligibility requirements.* Excellent communication, organizational, and leadership skills.* Ability to work independently, manage multiple cases, and collaborate with diverse healthcare teams._Please note- The annual base salary range is $130k to $140k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
