Bilingual Spanish/English MSW Case ManagerThe Bilingual Spanish/English MSW Case Manager is responsible for the following:Key Skills: Proven ability to manage documentation, strong organizational skills and practices, comfort and effectiveness visiting families in the community and in their home setting, demonstrated ability to establish productive rapport with families, and the ability to appropriately handle difficult topics. Successfully conduct outreach in the community and schedule interview appointments in the office and community settings.Desirable: Knowledge of DHS, Early Intervention, community supports and general services as well as current standards/recommendations for prenatal care and infant care & development, early intervention, community supports and general services.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.RESPONSIBILITIESPreliminary review of participant information received and successfully completes intake to enroll clients in BBO/FCM Program as directed by supervisor.Ensures that each participant has a Primary Care Provider and if not, link them with the appropriate medical services.Develops/maintains and updates comprehensive individual care plans.Conducts outreach at service sites and makes visits to participants' homes as necessary and/or required by program specifications.Completes internal time and activity in Cornerstone for submission to funding sources.Follows guidelines of case confidentiality according to agency standards.Maintains case records on his/her caseload and provides information on cases as necessary or required.REQUIRED EDUCATION AND EXPERIENCEMinimum: Masters Degree in Behavioral/Social Science, Social Work or a related field or Registered Nurse.NOTE: If you earned your degree from a college or university outside the United States, a copy of a Certified Translation of Degree must be provided.Bilingual (Spanish/English) preferred.Exposure and experience working with pregnant women, families, and children 0-6 years of age.Experience with community health, maternal and child healthcare.1 year of experience in data entry and data processing.Ability to establish good rapport with families and handle difficult topics.Must be computer-literate.Strong English language skills.Excellent telephone, interpersonal and written communication ability.Must provide own reliable transportation to travel to clients in the CAU geographical area as needed or required.The CAU Family Case Management program geographical area covers Chicago (primarily north of Roosevelt) plus the West, Northwest and North suburbs of Cook County (primarily north of I-55).
