~*Speech Language Pathologist*~Heritage is seeking a *consistent PRN SLP* or *Part-Time SLP* to cover 10-20hrs per week at our Lebanon location. Our *Part-Time* position offer *PTO accrual* as well as competitive compensation. The ideal candidate should have experience in a skilled nursing facility as well as being comfortable working w/ trach and vent patients.*Join our in-house therapy team! *Heritage Rehab provides high-quality physical, occupational, and speech therapies and rehabilitation at our Embassy Healthcare communities.We strive to help patients reach their maximum potential so they can live as independently as possible. Our experienced therapists work closely with patients and their caregivers to build customized rehabilitation programs tailored to each individual's needs.Our facility in Lebanon is 79 beds conveniently located off I-71 or I-75 and is easily accessible from both the Dayton and Cincinnati areas. The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient. The therapy gym is spacious and equipped with all the tools that a therapist needs to provide the most effective treatment interventions.Master's Degree in Speech Language Pathology from an accredited program Valid license to practice in Ohio.*POSITION DUTIES AND RESPONSIBILITIES:*1. Assigning patient care activities to professional and non-professional personnel.2. Interpreting responsibilities and SNF policies to therapy personnel.3. Assures the execution of doctor's orders.4. Oversees the maintenance of the therapy records (medical, personnel, accounting, billing, etc.)*PRIMARY THERAPY RESPONSIBILITIES:*1. Evaluates and treats communication disorders, including motor speech disorders, language disorders, cognitive language deficits and voice disorders.2. Provides direct patient services to improve the cognitive, communicative and or swallowing abilities of neurological impaired persons.3. Evaluates, treats and provides case management for dysphagic patients.4. Provides documentation to maintain quality of the medical records, as it relates to reimbursement guidelines, accuracy of billing information and facility requirements.5. Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice.6. Follows administrative policies and procedures related to emergency procedures, infection control and safety.7. Coordinates and or participates in training programs for facility staff, residents, families and rehabilitation staff.8. Initiates new program development with focus on functional outcomes to improve the quality of life of the people residing in the facility.9. Participates in continuous quality improvement activities.10.
