*Gotham is seeking highly skilled and compassionate UAS Nurse Assessors to provide person-centered assessments in New York State. This role involves conducting initial assessments and reassessments for individuals requiring Long-Term Care Services, ensuring compliance with the latest NYS guidelines and regulatory standards. Our nurses work to support the well-being of clients, helping them thrive in their homes and communities. **/*/* ONLY INITIAL ENROLLMENTS AVAILABLE /*/***Work Schedule: ** Minimum of 5 cases per week*Compensation: ** Fee for service: $200 per initial enrollment visit*Key Responsibilities: ** UAS Assessments: Conduct initial and reassessments in line with NYS UAS standards, identifying eligibility for long-term care services.* Compliance: Ensure compliance with NYS regulations, including adhering to latest guidelines on Medicaid/Medicare and Managed Long-Term Care (MLTC) plans.* Data Entry & Documentation: Accurately document assessments, concerns, and conflicting information within the UAS evaluation system. Promptly enter assessment data into the electronic UAS platform.* Client Advocacy & Care Planning: Provide educational guidance and healthcare resource options to clients and their families, ensuring informed decision-making regarding long-term care.* Interdisciplinary Collaboration: Work alongside interdisciplinary teams to enhance patient care and continuity, improving overall service quality.* Quality Assurance: Maintain up-to-date UAS modules and participate in continuous learning regarding the assessment system and any changes in best practices.* Regulatory Updates: Stay informed of evolving UAS assessment protocols and implement best practices, ensuring that all assessments meet NYS Department of Health requirements.* On-site Visits: Perform all evaluations in-person, either in the client's home or facility, following NYS requirements for in-person assessments.*Qualifications: ** Active RN License in NYS: Must hold an unencumbered New York State RN license and a valid NYS identification.* UAS Certification: Must have an active UAS User ID number.* Experience: Minimum of 1 year of UAS/home care assessment experience, with familiarity in MLTC and Medicaid/Medicare plans.* Skills: Strong clinical assessment, critical thinking, and documentation skills.* Tech Proficiency: Experience with electronic health records (UAS evaluation forms) and QA tools (e.g., Lenavi & QAS) is preferred.* Communication: Excellent verbal and written communication skills to effectively interact with patients, families, and interdisciplinary teams.* Best Practices Knowledge: Deep understanding of best practices related to UAS assessments and a commitment to keeping certifications and knowledge current with NYS standards.Job Types: Full-time, Part-timePay: From $2,000.00 per weekBenefits:* Health insurance* Paid time offMedical Specialty:* Home HealthApplication Question(s):* Have you completed a UAS assessments/reassessments within the last 2-4 months?* Are you up to date with all modules within the UAS system?* Please provide your UAS ID # (Must be provided for consideration)* What boroughs are you willing to cover: Only Queens or Manhattan (Please Specify)Experience:* UAS Assessments: 1 year (Required)Language:* Spanish (Preferred)Work Location: On the road
