The LPN works as part of the nursing team to assess patient needs, plan and modify care and implement interventions.ESSENTIAL JOB FUNCTIONS include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Provides direct resident care and treatments, including, but not limited to:Prepares, administers and records medications in accordance with facility policyMonitors and records temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, catheterizations, etc.Sterilizes equipment and supplies as neededCollects samples such as blood, urine and sputum, and performs routine laboratory tests on samplesPrepares residents for medical treatments, examinations and tests, explaining the procedures and their purposeAdministers prescribed emergency careAnswers residents' call-lights and determines how to assist them as neededAssess residents' conditions and vitals to determine and/or modify resident treatment plansMonitors, records and reports symptoms and changes in residents' conditionsProvides input for the development of individualized care plans and implements accordinglyMaintains accurate, detailed reports and recordsRecord residents' medical information and vital signsDocuments nursing history and physical assessment for assigned residentsUnderstands and reviews the facility's Healthcare Clinical Policies and Procedures and Safety GuidelinesReports any issues or problems that may arise to the Director of Nursing and/or AdministratorComplies with state, federal, and all other applicable health care and safety standardsAssists families and other visitors as neededAttends/completes required in-services and other required meetingsPerforms other duties as directedEDUCATION and EXPERIENCE an equivalent combination of education, training and experience will be considered.Graduate from an accredited nursing program; minimum of 1 year of previous long-term care experience; or an equivalent combination of education and experienceMust be licensed in the state of Illinois as a Licensed Practical Nurse (LPN)Benefit Package Includes:Excellent PayFlexible SchedulingPaid Vacation Time-Cash Out OptionPaid Sick TimeGroup Health Insurance / Dental & Vision / Life Insurance / Disability / RetirementPaid BirthdayOur company provides equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, sex, national origin, age, disability, or genetics.
