Position: Lead LPN (Licensed Practical Nurse)Location: TapestryPay: DOEShift: Mid shift (EOW)Job purposeUnder the supervision of the RN Nurse Supervisor, the Lead LPN performs as a licensed care giver and assumes responsibility and accountability for Patients' well-being while providing assistance to the Nurse Manager as a leader among its peers.Duties and responsibilitiesThe following duties are routine for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.Adhere to all facility policies and procedures as well as all DEA, BOP, MDH and DHS regulations.Provide appropriate nursing services and education to patients/clients.Consistently communicate pharmacy and nursing related issues and activities with RN Nurse Manager.Observe, record, and report, to the appropriate person, any symptoms, reactions and changes in a patient's condition.Receive and transcribe physician orders to patient/client charts.Chart nursing notes in an informative and descriptive manner reflecting the care provided and the response to that care and assist another nursing staff with the same.Administer Mantoux to patients/clients and staff as required.Attend and participate in staff in-services and meetings.Assist Nurse Manager with audits, staff education, orientation of new nursing personnel and daily activities as requested.All other duties as assigned.The following duties are normal for this position at Residential locations.Obtains blood specimens by performing venipuncturesReceives and transcribes physician order to patient/client charts.Participate in report at shift change.Complete scheduled weekly updates.Complete scheduled chart audits.Maintain complicacy with the Minnesota Department of Health regulations.Attends and participates in staff in-services and meetings.Nurses may not admit, transfer, or discharge clients. They may be directed to follow through with the process of implementation of the above by the director or designeeThe following duties are normal for this position at MAT locations.Dispense medication as prescribed.Maintains absolute accuracy in daily accounting of all medications that are dispensed and in inventory.Provide ongoing training to dosing nursesAct as the point person for pharmacy, dosing software, policy/procedure and nursing related issues and respond in a supportive manner.Conduct blood draws as required.Administer EKG's as requested.Lead nurses are responsible for filling Suboxone carry outs.Monitor and order Subutex and Suboxone as needed.Maintain bins for new and discharged patients.Monitor for phase changes and specials and adjust carry outs prepped accordingly.Monitor for dose changes and adjust doses accordingly.Monitor for absent patients and adjust doses based on absent day protocol.Fill and verify Suboxone doses for the upcoming three days, as required.Obtain verification from PA of prepped doses.QualificationsMust have LPN licenseMinimum 1 yr experience in a chemical dependency facility and2-3 years related experience required, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.Must know and understand all rules, regulations, and requirements of the assigned program.Must have two years continuing freedom from chemical use problems.Must have 12 hours of training in co-occurring mental health problems and chemical abuse of dependency that includes competencies related to philosophy, screening, assessment, diagnosis, and treatment planning, documentation, programming, medication, collaboration, mental health consultation, discharge planning.If currently employed for Meridian, must be free from any Corrective Action Plans.CPR and first aid certified.Required Skills, Knowledge, and Ability· Strong verbal skills and ability to communicate abstract concepts in a simple format· Ability to communicate with a variety of people from diverse cultures, socioeconomic and educational backgrounds.· Flexibility to meet a variety of changing priorities· Computer skills to draft emails, memos, and reporting· Cultural awareness and demonstration of sensitivity to various cultural norms and valuesPhysical RequirementsHearing 100% Ability to hear staff, incoming calls, and callersVision 100% Ability to see computer screen and written charts to documentoutcomes and obtain dataSpeaking 100% Ability to speak to staff and reviewersStanding 6-8 hours per dayBenefits:Twenty (20) days of paid-time-off + 6 paid holidaysHealth, dental, and vision insurance + HSA/FSA401(k) - ROTH and traditional optionsCareer path opportunitiesDiscount with Verizon cell phone plansTuition reimbursement and discounted tuition at select collegesA job with a sense of purpose as you help people on their path to recoveryWe owe our success to the passion, commitment and expertise of our staff.
