We are excited to be on the search for a PRN Speech Therapist to join our dedicated team at The Bellefontaine Healthcare Center! We are located in Pasadena, CA and known for our respective partnerships in the area. Our facility has a total of 130 beds in both skilled and subacute with 4-Star overall CMS rating.The ideal candidate will have experience within the Skilled Nursing setting with a minimum of 1 year. The SLP will carefully assess patient care and create a treatment plan for the respective residents. You will work closely with the DOR, OT, and PT to create a treatment plan and document the progress of each resident.About the Position:The SLP will provide all aspects of quality patient care. This includes evaluation, treatment, care plans, family education and documentation.Communicates patient status and needs to the patients, family, caregivers, or other members involved with patient care.Evaluations are initiated per M.D. orders-payer authorization and facility/payer source guidelines.Develops treatment plan and provide treatment, consistent with benefit and reimbursement level for best functional outcome for patient.Educates staff, other health care professionals and third party payer on the benefits and functional outcomes of intervention.Appropriately and effectively completes documentation including evaluations, daily progress notes, summaries, or monthly reports as required.Screens all individuals who may benefit from therapeutic intervention.Uses professional judgment to ensure safety of self, patients and others at all times.Achieves quality patient care through interdisciplinary co-op, co-treatment, setting up protocols and programs.Confers with Program Specialists and supervisor to develop and implement new programs/techniques to enhance program utilization.Keys information into computer systems, including EMR and rehab systemsQualifications:Graduate of an approved curriculum in Speech and Language Pathology.Is in good standing with all regulatory agencies and licensing boards.Current, active license as Speech Pathologist, and is able to practice unencumbered.CPR/BLS Active CertificationSkilled Nursing Facility strongly preferredSchedule:PRN as neededPay:$60-65/hour Depending on ExperienceWe provide equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, creed, sex (including pregnancy, childbirth, or related condition), age, national origin or ancestry, citizenship, disability, marital status, sexual orientation, gender identity or expression (including transgender status), genetic predisposition or carrier status, military or veteran status, familial status, status as a victim of domestic violence, or any other status protected by law.
