We are currently seeking a: Clinical Nurse LPN RN Specialist - Home HealthAs a non-profit 501(c)(3), Kline Galland employees are eligible for the Public Service Loan Forgiveness Program. Offering relief on student loans!This newly created role is essential to provide telehealth support and virtual visits (from our office) to our patients, as well as bridging the gap between intake, clinical care, and quality assurance. You will play a key role in ensuring high-quality care and optimal patient outcomes within our Medicare-certified home health agency.Responsibilities:Telephonic support: Provide expert clinical guidance and education to nurses, therapists, and aides via phone consultations, addressing patient needs and concerns, and offering timely interventions.Virtual visits: Conduct comprehensive virtual assessments and follow-up visits with patients, utilizing technology to monitor progress, manage care plans, and document interventions.Quality control: Analyze patient data and clinical documentation to identify areas for improvement, implement quality initiatives, and ensure compliance with regulations and best practices.
