Hospice Social Worker - MSW, home visits, full-timeFaith Hospice is seeking a full-time master's prepared Social Worker to join our community team!!What will you do?Work with a manageable case load within an assigned manageable geographic territoryProvide support to patients, families, and caregivers by offering emotional support, information, education, long-range care planning, short-term counseling, and assistance with identifying and accessing resourcesMonday-Friday with minimal on-call rotation 3-4 weekdays per monthEducation and Skills to be successful:Master's degree in Social Work as well as full licensure from the State of Michigan as an MSW (limited license is accepted with the requirement of continuing to work towards full licensure while employed by Faith Hospice).Minimum of one year of medical social work experience in a health care/medical settingStrong knowledge of community-based resourcesPrior hospice social work experience is highly valuedCandidates must also have proof of Michigan driver's license and vehicle liability insurance as well as a reliable means of transportation to make home visitsIf this rare opportunity calls to you, please apply on-line at click on the Careers tab.
