The Shelter Nurse Practitioner will provide comprehensive primary care and healthcare services to individuals with HIV/AIDS and/or experiencing homelessness in a supportive shelter environment. This role is vital in addressing the complex health needs of shelter residents, including those with chronic conditions, mental health issues, substance use disorders, and acute illnesses. The Shelter Nurse Practitioner will work collaboratively with a multidisciplinary team to ensure high-quality, compassionate, and accessible healthcare to vulnerable populations.*Responsibilities:** Conduct thorough health assessments and develop individualized care plans for shelter residents.* Diagnose and treat acute and chronic medical conditions, including but not limited to infections, wounds, respiratory issues, and chronic diseases (e.g., hypertension, diabetes).* Prescribe medications, order labs, and interpret diagnostic tests as needed.* Provide preventative care, including immunizations, screenings, and health education.* Provide mental health assessments and collaborate with mental health professionals to address co-occurring disorders.* Offer education on health promotion, disease prevention, and self-care management.* Work as part of a multidisciplinary team, including social workers, case managers, mental health professionals, and shelter staff, to address the holistic needs of residents.* Collaborate with community health organizations, public health agencies, and external healthcare providers for referrals and continuity of care.* Maintain accurate and up-to-date patient records using electronic health records (EHR) or paper systems, ensuring all medical documentation is complete and compliant with healthcare regulations.* Monitor and report on health trends within the shelter population to inform staff and adjust care protocols.* Provide health education to residents on topics such as nutrition, hygiene, sexual health, substance use prevention, and chronic disease management.* Advocate for the health and well-being of shelter residents, ensuring they have access to necessary resources, including mental health support and specialty care.* Respond to medical emergencies and provide crisis intervention as needed, stabilizing residents and facilitating transport to emergency services when necessary.* Support shelter staff with health-related emergencies, ensuring a safe and supportive environment.* Participate in continuous quality improvement (CQI) initiatives to improve the overall care provided to shelter residents.* Contribute to policy development and protocol review to enhance healthcare services within the shelter setting.*Requirements:** Master's degree in Nursing (MSN) from an accredited Nurse Practitioner program.* Current, active Family Nurse Practitioner or Adult Gerontology Nurse Practitioner license in the state of New York.* Experience with the HIV/AIDS population preferred.* At least one year of primary care experience.---_Please note- The annual base salary range is $125k to $150k.
