Per Diem Weekend NurseCommunity-based, mission-driven hospice seeks a Per Diem Weekend Nurse to join its expanding On-Call team. The position includes weekend availability. Travel is required to provide care in patients private homes and residential facilities throughout Greater Boston and parts of Metrowest. The ideal candidate is well-organized and enthusiastic about working in a fast-paced environment.Job Description:The Per Diem Weekend Nurse is an integral part of an interdisciplinary hospice team responsible for caring for patients at the end of life. Makes home and hospital visits, provides information sessions, evaluates new patients, facilitates admissions to hospice services and coordinates care planning. Employs triage and assessment skills to titrate medications and manage symptoms appropriately and provides education to patients and families about terminal disease and end-of-life process.
