If you are passionate about reshaping healthcare and want to work for a mission driven organization where new ideas and innovation are valued, then we would like to meet you.*About Your Work*The Health Navigator assists members in navigating and connecting to clinical and community resources. Supports clinical staff in identifying and providing outreach, and baseline assessment services to members. Establish and maintain effective, ongoing relationships by facilitating communication and coordination with members, their caregivers, PCPs, and other providers. Connects members with health screening programs, and other resources. The Health Navigator will focus on care coordination and short-term case management for members needing support in accessing medical, social or behavioral services.This position is located in Los Angeles and will require on-site hospital visits throughout the LA region.*You will focus on:** Under general supervision and guidance from the CM leadership team, the Navigator supports clinical staff through the completion of components of case management and disease management programs.* Assist members in getting appointments & access to appropriate health care and community programs.* Provide one to one guidance, support, education, coordination of care and other assistance to members as they move through the healthcare continuum.* Initiate follow-up to confirm and coordinate additional needs.* Participate in case conferences and meetings with the CM team and medical directors to support effective care coordination.* Effectively establishes rapport with team members, patients/families, and physicians.* Ability to provide a high level of customer service to members, internal and external customers in a professional, service-oriented, manner* Educate and answer inquiries from members and or their caregivers about benefits, services, eligibility and referrals with a positive and professional approach, promoting member satisfaction and retention.*The Skills, Experience & Education You Bring** High school diploma or equivalent education, experience or training required.* Minimum Experience: two (2) years in an acute care facility, preferably case management or emergency department.* Preferred: Medical Assistant, CNA, Home Health Aide, or similar certification* Strong organizational skills.* Strong verbal and written communication skills.* Ability to effectively cope with stressful situations, manage, multiple, and sometimes conflicting priorities simultaneously.* Knowledgeable of managed care, including contracted and non-contracted providers.* Spoken and written fluency in English.* Maintains confidentiality of all PHI in compliance with state and federal law, and BTP policy.*You Share Our Mission & Values:** You are *passionate* about improving the healthcare experience and want to be part of the Altais mission.* You are *bold* and *curious*- willing to take risks, try new things and be creative.* You take pride in your work and are *accountable* for the quality of everything you do, holding yourself and others to a high standard.* You are *compassionate* and are known as someone who demonstrates emotional intelligence, considers others when making decisions and always tries to do the right thing.* You *co-create*, knowing that we can be better as a team than individuals.
