The Home Care RN uses the nursing process of assessment, problem identification, planning, intervention, and evaluation.Essential Job Responsibilities of an RN:Provides skilled nursing care based on a compassionate and holistic approach.Updates a comprehensive care plan that addresses physical, emotional, sociological, and spiritual needs of the patient/family in conjunction with the patient/family and program staff.Advocates for pain/symptom control.Provides telephone triage support to Clinical Team.Teaches, supervises, and counsels the patient and family regarding care concerns.Directs and supervises LPNs and Hospice Aides, as requested.Maintains close communication and involvement with the attending physician and IDT members involved in the patient's care.Performs admission of patient to hospice care when needed.Qualifications of an RN:Required Knowledge, Skills, and Abilities of an RN:Familiarity with community health organizations, practices, and resources.Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.Demonstrates general technical abilities with nursing functions, such as, IV administration, equipment function and maintenance.Ability to plan, assess, implement, and evaluate nursing care for Hospice of Lenawee patient/families.Ability to function autonomously in a home setting.Education/Credentials of an RN:Graduate of approved School of Nursing.Current Michigan Registered Nurse License.BSN degree, preferred.CPR certification, required.Experience of an RN:Two years nursing experience in hospice, home health, or oncology nursing, preferred.Experience with dying patients and their families, hospice experience preferred.Experience in health care, required.Additional Requirements of an RN:Valid driver license and automobile with current insurance.Position Status/Classification: Non-exempt, Hourly, Regular, Patient CareVisit our website here:our Hospice of Lenawee Heroes Video:
