To Apply for this Job Click HereAmerican Medical Staffing (AMS) puts you first, and we are currently seeking a Hospice Licensed Clinical Social Worker for a contract Home Hospice position in Worcester, MA.Hospice LCSW Job Details:Pay Rate: $2,200-$2,400 gross per weekSchedule: 8:00AM-4:30PMAvailable Shift: 5 shifts, Monday- Friday each weekStart Date: 11/25/2024Located In: Worcester, MARequirements: Qualified applicants MUST have at least 2 years of experience in hospice, have a valid Social Worker license and be willing to obtain MA licensure.As a Hospice Social Worker, you'll have the opportunity to provide comprehensive assessments to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. As their in-home social worker, you will conduct comprehensive assessments of patients and develop individualized care plans in collaboration with their interdisciplinary team.In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum. This mobile role offers independence and differentiation in your day-to-day work, while also keeping you connected to the coordination of care with other healthcare providers like physicians, therapists, and nurses.What are you waiting for?
