This position is expected to improve access and be the point of contact primarily responsible for initiating services.The assessments conducted occur mostly in client's homes, though they could take place in schools or other community locations. The client population served would be children, adolescents, or adults with behavioral health challenges.Conduct Comprehensive Needs Assessments (CNA)Meet face to face with the identified client and guardian (if the client is a minor) to gather relevant clinical informationComplete intake forms (including appropriate releases)Write the narrative of the CNA, as outlined in the EHR formPopulate the client's profile in the EHR (including diagnoses, medications, allergies, etc.)Connect with associated professionals who are/have been involved with the assessed clientRecommend service(s) based on the CNAComplete referral management workflowConduct the service authorization processComplete the Service Authorization Form in the EHRSubmit the form to the appropriate MCO (via efax or MCO portals)Interact with the MCO Care Coordinator (if necessarily) regarding addition information requests or service justificationNotify local leadership and the NCG authorization specialist about completionOther Activities:Responds timely to all communication (emails, voicemails).Proficient in G-SuiteParticipate in mandated team meetings and case staffings with Supervisor TeamsMaintains office hours to be available to support clinical teams with completing assessments and/or authorizations (for assessments not completed by the Clinical Assessor), reviewing portals for authorization approvals, and uploading or addressing relevant assessment and authorization documents in the EHRAble to be responsive and available to meet dynamic needs of clients and clinical supervisor staffEducationRequiredMasters or betterLicenses & CertificationsRequiredRes in Counseling (VA)PreferredLicMarriageFamilyTherapyLicense Prof CounselorLic Clin Social WorkerBehaviorsPreferredTeam Player: Works well as a member of a groupDedicated: Devoted to a task or purpose with loyalty or integrityEnthusiastic: Shows intense and eager enjoyment and interestMotivationsPreferredSelf-Starter: Inspired to perform without outside helpGoal Completion: Inspired to perform well by the completion of tasksAbility to Make an Impact: Inspired to perform well by the ability to contribute to the success of a project or the organization
