RN Case Manager | Hospice | $5000 Sign On BonusThe Hospice RN Case Manager establishes a professional and caring relationship with our patients and families, support staff, physicians, facilities and other health care professionals by providing hands-on-care, on-going clinical management and education regarding hospice care.As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.Additional responsibilities of our Hospice RN include:Making initial nursing evaluation visitContinually evaluating the patient's needsInitiating, coordinating and revising the treatment planRendering nursing care and performing treatments, as ordered by the physicianParticipating in the Quality Assurance/Quality Improvement processPreparing accurate, orderly and timely reports and documentationAdhering to all practice standards as they apply to patient careRequirements:Caring, patient and flexible teacher with excellent communication and time management abilitiesPossess independent problem solving abilities, excellent clinical assessment skillsComputer literateGood physical shapeAbility to lift/transfer patients to and from wheelchairs, if necessaryAssociate's of Science degree in NursingValid Ohio RN licenseMinimum 2 years of nursing experience in hospice preferredValid driver's license and one year clean driving recordExcellent writing, communication and charting skillsWe're excited to share that we've been named a 2024 NorthCoast 99 award winner!
