Overview:Join Our Growing Team!At the Visiting Nurse Association of Ohio, we're seeking compassionate and dependable individual to join us a RN Case Manager - Mental Health in Western Cuyahoga and surrounding areas. The RN will be responsible to function independently in providing comprehensive care to patients who are experiencing mental illness both acute and chronic disease states. Join us and be part of a meaningful mission to enhance patient care.Responsibilities:Key Responsibilities:Identifies needs and nursing diagnosis, assigns priorities to plan of care based on holistic approach.Utilizes discharge outcome criteria to evaluate the plan of care for each patient.Individualizes the health care plan of care for each patient. Appropriately utilizes pre-printed care plans.Demonstrates and maintains clinical expertise in providing care to the patient.Takes responsibility for identifying unmet learning needs and seeks assistance from supervisor and peers to maintain currency with nursing skills.Utilizes appropriate psychiatric nursing skills to meet the unique needs of each patient.Provides for client participation in health promotion, maintenance and restoration.Collaborates with community agencies to coordinate patient care and facilitate discharge planning.Meets with the Manager at least once per month to communicate status of patients and concerns.Utilizes principles of the nursing process to assist in data collection, planning and evaluation of nursing care.Qualifications:Qualifications and Requirements:Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree. Bachelor's degree in Nursing preferred.2+ year's experience in mental health nursing.1 year experience in medical surgical nursing preferred.Experience in home health care.Bi-lingual a plus.Required Licenses, Registrations and/or Certifications:Registered Nurse, licensed in the State of Ohio.CPR certification is required.Valid driver's license.Mental Health requires initial approval/certification by Medicare fiscal intermediary (Palmetto).
