_*Salaried Position*_*Position Summary:*Physical Therapist provides therapy services to patients in a home health environment by assessing patient needs and generating treatment programs with established goals.Position Responsibilities for a Home Health- PT:/* Evaluates patients to assess patient needs, conducts assessments necessary for creation of patients individual Plan of Care./* Plans and implements individualized treatment program by identifying short and long term goals and methods to achieve identified goals./* Supervises and directs licensed Physical Therapist Assistants in the implementation of individual patient's POC./* Reviews the quality and appropriateness of the visits delivered for effectiveness and efficiency through co-signing of PTAs visit notes./* Provides treatment in accordance with established standards of practice./* Consistently demonstrates sound judgement in evaluation, planning and implementation of patient therapy programs./* Communicates in a positive, clear, professional manner with patients, families and other healthcare team members, especially when providing education.Knowledge, Skills and abilities: Must meet all certification and licensure requirements as established by the state and national board associations.
