Port Healthcare CenterRegistered NurseFull-Time, 40 hours/week - all shiftsBase Pay: $40.39 - $48.00Differentials offered for evening, night and weekend shifts:Essential Duties: To provide exceptional care to all residents including but not limited to:- Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary- Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary- Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action- Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily- Performs comprehensive assessments of each resident on unit- Reviews and approves admission and re-admission nursing assessments completed by an LPN- Develops care plan based on each resident's assessment- Obtains physician orders and any revisions for all treatment and medications needed- Assesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of condition- Renders immediate assistance to any resident in need- Supervise and evaluate all direct resident care and initiates corrective action as necessary- Provides clinical supervision of nursing assistants providing direct patient care- Assumes responsibility for compliance with federal, state and local regulations within the assigned unit- Counsels nursing assistants and communicates clearly with Clinical Nurse Managers and/or Director of Nursing if issues arise- Collaborates with interdisciplinary team to assure that high quality resident care is maintained- Prepares nurse assistant assignments and explains all pertinent resident care- Supervises distribution of nourishments- Supervises the removal of all postural supports- Orders, receives and documents medication according to facility procedure- Supervises response to residents calls for assistance and responds to call lights as necessary- Insures that all physician renewal orders and physician documentation is timely and appropriatePrerequisites:- Graduate of an accredited Registered Nurse school of nursing- Current and active RN license in the state of Massachusetts- Current certification in Basic Cardiac Life Support in accordance with AHS or ARC standards- Knowledge of Geriatric Nursing required9BJIe0T4FO
