*CCBHC- Registered Nurse/Health Coordinator**JOB TITLE:* Registered Nurse/ Health Coordinator*REPORTS TO:* Medical Director and Program Director*DEPARTMENT: * CCBHCWe believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project.Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:* Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization; Screening, assessment, and diagnosis including risk assessment; Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning; Outpatient mental health and substance use services; Outpatient clinic primary care screening and monitoring of key health indicators and health risk; Targeted case management; Psychiatric rehabilitation services; Peer support, counselor services, and family support services; and, Intensive community-based mental health care for members of the armed forces & veterans.*SUMMARY: *The Registered Nurse works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing a serious mental illness and/or substance abuse disorder, and a chronic health condition. As a member of the client care team, the RN will establish therapeutic relationships in order to facilitate client progress with treatment goals, administer vitals, and conduct health education intervention. To be effective, the RN will have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers and work with a multi-disciplinary team to engage, establish and achieve appropriate treatment goals.*JOB DESCRIPTION: ** Conduct CCBHC program required health screening, including but not limited to, vitals at intake and a follow up annually.* Conduct and document a medical assessment within 30 days of patient admission and determine the need for a physical examination and medical care.* Review physical examination results provided by patients from outside medical practitioners.* Provide health education and appropriate medical referrals for all patients receiving treatment in the outpatient chemical dependency program;* Conduct and document PPD Screenings as needed and determine medical necessity for additional tests and procedures and review results, including referral for chest x-rays to verify PPD results, and advise clinical staff regarding the patient's approval for participation in treatment services.* Conduct and document a Communicable Disease Risk Assessment and/or review and approve those completed by clinical staff on all patients, to include education, risk reduction, counseling and referral regarding HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and other communicable diseases;* Review, approve and sign treatment plans and participate in the Multidisciplinary team in order to advise and provide oversight of all patient's medical care while enrolled in the outpatient chemical dependency program;* Provide medical trainings to staff as instructed by the Directors, including training and monitoring staff provision of Communicable Disease Risk Assessment, and provision of Infection Prevention and Control training; and* Responsibility to ensure and document partnership/collaboration with community partnerships and collaborations to enhance service delivery and access.* Perform other work-related duties as assigned.* Available to weekend hours or evening hours as required.* Work collaboratively with the CCBHC Interdisciplinary Team as well as external agencies and community stakeholders.* Attend all required management and staff meeting and complete all CCBHC and organizational trainings.*EXPERIENCE: ** Extensive knowledge of nursing services.* Experience with adults with mental health and/or substance abuse needs.* Knowledge of intake and referral processes; knowledge of community resources.* Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources.* Well-organized with strong time management skills.* Excellent written and verbal communication.* Able to work independently and as a member of a team.* Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.* Proficient with word processing, electronic health records, and electronic communications.* Demonstrated competency in written and verbal skills.* Able to meet program standards in documenting in client records.* Ability to work with challenging clients.* Comfortable with community outreach and home visits.* Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.* Bilingual skill in a language other than English valued.*QUALIFICATIONS: ** Registered Nurse with at least 2 years of professional experience.* New York State Licensure.* Experience in the provision of medical assessments, coordination of medical services, and administration of PPD screenings.* Experience in the chemical dependence and mental health clinics preferred.*BENEFITS WE OFFER: ** Medical including Dental and Vision* Cafeteria Plans: FSA, Dependent child care plan, qualified transportation plan* Up to 3% 401k Profit Sharing plan automatic contribution for eligible employees* 401k Additional 4% match of your contribution from your salary* Employee Assistant Program Services (EAP)* Community Mental Health Loan Repayment Program for qualifying candidate* Paid time off (PTO)* Referral Bonus*SALARY RANGE*: $105,000 to $115,000 *(*Based on experience and licensure/certification.)Interborough is an EEO employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regards to race, color, religion, sex, national origin, disability status, genetics, protected veterans' status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state and/or local law and encourage women, veterans and other minorities to apply.Interborough Developmental and Consultation Center is committee to Diversity, Equity, Inclusion and Belonging (DEIB) in the workplace.
