Social Worker (LMSW/LCSW)

The *Social Worker (LMSW/LCSW)* is responsible to provide social work services to meet the consumer's needs._ *Monday - Friday 8a-4p*_*Duties and Responsibilities** Assesses consumers and completes appropriate sections of the Comprehensive Functional Assessment for consumers annually.* Completes Initial Psychosocial Assessment and Comprehensive Psychosocial history as needed.* Develops, writes, implements and reviews social work interventions as necessary to meet the consumer's needs.* Develops and provides in-service training for staff in regards to social work and social work interventions.* Supervises social work assistants in regards to responsibilities by reviewing and signing off on work as required.* Provides outreach to consumers, family members and/or other people with whom the consumer is involved.* Coordinates with Accounting office efforts in regards to consumer financial resources.* Participates as a member of the Interdisciplinary Treatment Team.* Participates in consumer admission, movement and discharge procedures as needed.* Participates in projects and committees as assigned.* Attends and participates in Clinic and department meetings as necessary.* Provides consultation for staff when requested in regards to suggestions, ideas and information as to more effective means of dealing with consumers.* Interacts with consumers, clinical staff and other agency staff in a positive manner to facilitate the development of good working relationships and to enhance consumer relationships and their skill development.* Liaison between the consumer/resident/DT and those community resources utilized by the consumers such as day programs, workshops, hospitals, etc.* Ensures required information is maintained in both day habilitation and residential consumer records.* Assists family members in obtaining legal guardianship when appropriate.* Enforces and adheres to all safety policies and procedures.* All other duties as assigned.*Position Requirements and Qualifications** Master's degree in Social Work* Social Worker License* One-year experience working or training in the field of Human Services* Good oral and written communication skills* Good interpersonal skills* Valid NYS driver's licenseThe Social Worker (LMSW/LCSW) agrees to uphold IGHL's mission, emulate the values and commit to the vision.*IGHL Core Values**INTEGRITY:* We are committed to an environment that is authentic, fair, honest and operates ethically.*COMPASSION:* We treat each other with kindness and empathy, lifting up and supporting one another at all times.*TEAMWORK*: We value collaboration, open communication, and building a supportive environment knowing together we can achieve more.*INCLUSION:* We embrace diversity in people, ideas, and experiences where everyone is heard, appreciated and accepted.*RESPECT:* We value individual experiences and feelings while treating people with kindness and dignity.*VISION*IGHL's Vision is to be a Center of Excellence with state-of-the-art technologies and practices, embracing new business models and leveraging all of our resources.
