PRP Counselor

PRP counselors render individual and group services to adults in the home, community, and on-site environment. Counselors are required to conduct 6 visits a month which includes one visit a week for an hour. Counselors are responsible for implementing, documenting, and monitoring client progress towards their individualized rehabilitation plans.*Counselors help to rehabilitate clients in the following domains:**Self-care skills*--hygiene, nutrition, self-administration and compliance with medication;*Social skills*--community integration activities, interactive skills with peers and authority figures, developing natural supports, and supporting the child's participation in community activities;*Independent living skills*--maintenance of personal living space, community awareness, mobility skills, money management, and accessing available entitlements;*Cultural and spiritual activities--*Behavioral/Emotional self-management--conflict resolution, anger management, time management, maintaining age-appropriate boundaries.Promotion of illness self-management--education and information about mental illness, effective strategies to assist in managing illness, warning signs of relapse, helpful interventions.*Education and employment support **Disabilities *Counselors work in collaboration with all pertinent parties in the client's life, i.e. clinician, psychiatrist, DORS counselor, etc. All services delivered and data collected must be documented in the appropriate forms in a timely fashion.
