POSITION TITLE:Family Therapy Team LeaderPROGRAM: Functional Family TherapyGENERAL STATEMENT OF RESPONSIBILITIES:The FFT Team Leader is responsible for direct services with adolescents and their families including but not limited to evaluation/assessment; home & community based services; outreach; case and care management; family therapies; record keeping; collaboration with other professionals, team members and related community services (including consultation and/or community and/or school placements); and program management/supervision. The Team Leader assists in the supervision, development and implementation of assigned programs, services and staff.SUPERVISION: Supervised by: Director of Home Base Treatment Services (or designee)Supervises: Functional Family Therapy staff, Interns and/or staff designated by supervisor.EXAMPLES OF RESPONSIBILITIES:To provide comprehensive evaluations, treatment plans and treatment of adolescents and their families. This includes evaluation/assessment; home and community-based services; outreach; case and care management; and family therapies.To develop/implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation (only as per agency policy) advocacy and linkage to other needed services.To provide community-based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to: agency offices sites, client homes, schools, shelters, police stations, etc.Leads weekly clinical team meetings.Maintain contact and provide follow-up with families requiring this service. This will include daily home visits and home-based therapies.Maintain confidential and/or any other required program documents.Maintain a level of productivity and assure staff maintain a level of productivity as determined by the Executive Director and assigned by the program supervisor.To supervise assigned staff and/or interns.To assist the Director of Outpatient and Community Support Services and Executive Director in the day to day clinical and administrative management of assigned programs.To assist in program development, grant writing, marketing and community outreach.To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.To assist in the organization, marketing, planning and implementation of the program.Actively participate in Functional Family Therapy and internal training and staff meetings.To assure fidelity to Functional Family Therapy model of program staff.To serve on agency and/or community committees.Actively participate in supervision and report directly to the supervisor.Other such duties as assigned by the Supervisor.HP1EDUCATION & EXPERIENCE:A masters degree in marriage and family therapy or social work and a Connecticut license as a Marriage and Family Therapist (LMFT) or Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC),or a doctorate in psychology and a Connecticut license as a Psychologist is required.A Masters degree in a human service field with at least a one year clinical internship working with children and/or adolescents and a Connecticut license as a Professional Counselor will be considered.Two years post graduate clinical experience working with children, adolescents or families is required, four years experience is preferred.Experience providing clinical supervision and the necessary credentialing from NASW, AAMFT or the APA (or a similar credentialing body which approves supervisors or supervisors in training, status etc.) may be required. Experience in family therapies is also preferred. Experience with intensive in-home service model preferred.Valid driver's license, reliable transportation and valid auto insurance required.
