Under the Assistant Administrator of Special Populations and Marketing and Outreach, the primary purpose of this position is to coordinate, evaluate and facilitate referrals for potential admissions to Rutland Nursing Home (RNH) and Schulman & Schachne for Nursing & Rehabilitation (SSI). Will serve as a liaison to the nursing homes to help ensure a smooth transition with the admissions process between staff and applicants, hospitals and community agencies. Position is also responsible for ensuring admissions have appropriate reimbursement relevant to the Medicaid benchmark rates for specific service line. Responsibilities: Will consult with hospital staff to review and evaluate potential candidates for admission to the OBH SNF facilities. Assists in the coordination of referrals and admissions to RNH/SSI in order to maintain high occupancy. Demonstrates knowledge of admission criteria for all product lines. Collaborates with Admissions Department to coordinate admission transfers. Develops relationships with referring partners, including hospitals and agencies. Collects all required clinical and admission documentation in preparations for patient transfer to SNF. Review/analyze census reports for discharges, admissions, hospitalizations, and readmissions with follow up as necessary. Verification of payer source, i.e., Medicare, managed care, Medicaid, insurance. Demonstrates knowledge of Medicaid reimbursement/enrollment systems and ensures accurate payment structure for admissions. Develops relationships with Medicaid companies and agencies, including managed care companies and HRA. Interacts with HMO Case Managers prior to admissions to review skilled needs and appropriate level. Obtains authorizations for managed care organizations as needed. Weekend and holiday marketing/admission coverage as needed. Ability to navigate multiple referral systems which includes both internal and external referral platforms. Assists potential residents and their families by coordinating tours of the facilities and answer questions regarding the facilities and admissions process. Assists with inservices to referring partners and organizations. Maintains accurate documentation on all applicants evaluated in accordance with State and Federal regulations. Assists other departments with projects as needed in the spirit of teamwork. All other duties as requested. Qualifications: Education: Associate's Degree or higher preferred. Experience: Experience with and knowledge of Medicaid Systems (benchmark rates, conversions, authorizations) preferred. Experience working in post-acute admissions/marketing preferred. Knowledge and Skills: Good working relationships with staff and referral sources. Ability to multi-task and work accurately in a fast-paced environment. Bilingual is a plus. Thorough knowledge and understanding of medical terminology, conditions and treatments relevant to geriatric, young adult and pediatric populations. Ability to make thorough and accurate bio-psychosocial assessments of geriatric, young adult and pediatric applicants in relation to the continuum of services provided by RNH and SSI. Positive relationship building. Strong interpersonal and verbal communication skills. Organized with an ability to work independently to complete projects. Detail oriented with consistent follow-through. Excellent listening and problem-solving skills. Proficient with Microsoft Office suite applications such as Word and Excel. Physical Requirements: Ability to work in a variety of office settings. Position requires prolonged periods of sitting, reaching, walking throughout the work day. Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
