This position requires in-person attendance with a team at 1 Brooklyn clinic location with travel in Brooklyn in support of the CCBHC Project, Coney Island Mobile Crisis Services.*Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:*Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization; Screening, assessment, and diagnosis including risk assessment; Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning; Outpatient mental health and substance use services; Outpatient clinic primary care screening and monitoring of key health indicators and health risk; Targeted case management; Psychiatric rehabilitation services; Peer support, counselor services, and family support services; and, Intensive community-based mental health care for members of the armed forces & veterans.*SUMMARY: *Experienced clinician needed to join the CCBHC Mobile Crisis Team (MCT) along with a certified peer support specialist and mobile crisis team lead. The crisis response team is part of the CCBHC program, which is dedicated to integrating treatment for mental health, substance use and primary medical care in the Coney Island area. Our Mobile Crisis Team provides community based, street-level mental health intervention, engagement, and support services to people experiencing severe behavior crises. We offer a broad range of services including assessment, crisis intervention, counseling, and information and referrals to community-based mental health services. The MCT will also provide mobile crisis intervention, as well as office-based and telehealth crisis response. The overall program will provide care coordination, rapid mobile crisis intervention and ongoing treatment in mental health, primary medical care and substance use in the Coney Island community.*JOB DESCRIPTION:** Accompany MCT on field visits to provide psychosocial assessment to individuals in community settings (i.e., group homes, shelters, patients' homes, street, clinics) who are at exceptional psychiatric risk, including suicide.* Provides psychiatric crisis intervention, support and treatment in the community to help stabilize individuals and identify appropriate level of care needed. Provides clinical support to the Mobile Crisis Team, a team of three professions, in the field. Exercises considerable clinical judgment with respect to risk assessments and treatment of psychiatric patients in outreach settings.* Provides crisis intervention in person as well as via telehealth. When needed, meet walk-in patients from the mental health clinic who are in acute crisis.* Conduct intake, crisis and psychosocial assessments, including but not limited to mental status, suicide and risk assessments.* Functions as a therapist and contributes to the evaluation of an interdisciplinary group of staff. Participates in and supports the daily interdisciplinary treatment team meetings, which may include discussion of clinical cases, treatment planning and documenting minutes from those meetings. Provides coverage and accessibility for intakes, vacations, sick calls, as needed.* Supports the assessment of safety of scene prior to deployment of Mobile Crisis Team and accessing need for law enforcement services to ensure safety of Mobile Crisis Team in the field. Involves family and other mental health treatment providers as necessary in interventions.* Conducts brief individual and family psychotherapy interventions.* Collaborate with an interdisciplinary team which includes therapists, care coordinators, certified peer specialists, mobile crisis clinicians, psychiatric nurse practitioners and psychiatrists.* Work with care coordinators and patients to establish care coordination needs and add them to the patients' service plans.* Work with patient and patient's outpatient therapist when patient is in crisis to provide additional support.* Provide education to clients regarding their medical diagnoses and assist them to engage with their medical providers.* Communicates to the supervisor about the need for proper treatment and referral protocols with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of IDCC.* Participates in on-call activities of the MCT, including after hours with other duties as assigned.* Follow policies, procedures, and protocols to ensure compliance with CCBHC program requirements and regulations for substance abuse services.* Develops and implements treatment plans tailored to individual client needs for a person-centered approach.* Complies with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity.* Follow quality assurance processes to ensure the continuous effectiveness of clinical services.* Comply with all relevant regulations, accreditation standards, and reporting requirements.* Participate in audits and reviews to ensure adherence to program requirements and standards.* Complete agency training to improve cultural competency and trauma informed* care as they align to program requirements for CCBHC MCT and Agency specific mission.* Perform other work-related duties as assigned.* Available to travel/work evening and weekend hours as required by the grant and agency needs.* Work collaboratively with the CCBHC Interdisciplinary Team as well as external agencies and community stakeholders.* Attend required management and staff meeting. Complete CCBHC and organizational trainings.*EXPERIENCE: ** Experience - 3 years of experience working with clients with SMI or chronic substance use.* Mobile Crisis, ACT, Inpatient Psychiatry, or Emergency Room Psychiatry experience required.* Knowledgeable regarding the psychosocial aspects of illness and health.* Proficient with word processing, electronic health records, and electronic communications.* Well-organized with strong time management skills.
