Incumbent is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital.Responsibilities:Completes psychosocial evaluationsCompletes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan processConducts individual, group psychotherapy as prescribed by the treatment planParticipates in departmental meetingsDocuments according to clinical standardsDevelops after care plans for patients upon discharge including linkages to other agencies as appropriateProficiency in implementing Suicide Risk AssessmentProficiency in implementing Telehealth Services Policy in compliance with Federal andNYS regulatory guidelinesJob RequirementMaster of Social WorkValid NYS LCSWBilingual Spanish required2 + years of experience in Behavioral Health Outpatient settingIf you have the above skills, and want to hear more about this role, please reach out to Sheila Erickson, 860-982-7132
