Now, you have the opportunity to join us.The Ambulatory Care Network provides services such as triage, screening, referral and crisis intervention to ensure the mental well-being of our patients.You will be responsible for seeing patients virtually and in person, viascheduled appointments and walk ins. Position involves a wide range of responsibilities including but not limited to: crisis intervention, assessing for safety using evidence based screening, concrete services, assisting and following up with home care, psychosocial assessments, referrals to CBO's.
