The DayRehab & OP Care Manager will respond to requests from physicians and other clinic team members to ensure that patients are provided information and resources to assist their needs.The DayRehab & OP Care Manager will consistently demonstrate support of the Shirley Ryan AbilityLab statement of Vision, Mission and Core Values by striving for excellence, contributing to the team efforts and showing respect and compassion for patients and their families, fellow employees, and all others with whom there is contact at or in the interest of the institute.The DayRehab & OP Care Manager will demonstrate Shirley Ryan AbilityLab Core Attributes: Communication, Accountability, Flexibility/Adaptability, Judgment/Problem Solving, Customer Service and Core Values (Hope, Compassion, Discovery, Collaboration, and Commitment to Excellence) while fulfilling job duties.Job DescriptionThe DayRehab & OP Care Manager will:Interviews patients and family members to obtain information about home, social environment, family relationships, health history, patient's functioning and treatment goals.Explains patient's background, psychological factors, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.Collaborates with other health care professionals to develop, manage, and measure the effectiveness of the program.Assists patients and family with community integration and referrals to programs and services that will assist the patient in their home environment. Identifies eligibility and assists family in applying for entitlements. Investigates alternative sources of funding for uncovered services in the community.As clinically indicated, the DayRehab & OP Care Manager may provide psychotherapy services for patients to assist with the emotional reactions to their disability.Provide crisis intervention when necessary.Participates in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work.Maintains patient documentation per organization requirements.Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLabReporting RelationshipsReports directly to the Manager, Psychology.Knowledge, Skills & Abilities RequiredMaster's Degree in Social Work from an accredited university required.Current Illinois designation as a Licensed Clinical Social Worker (LCSW) required.Demonstrated understanding of the principles, practices, techniques and professional standards in the field of social work and psychotherapy.Knowledge of community resources, health care system and interdisciplinary functioning.Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.Working ConditionsNormal office environment with little or no exposure to dust or extreme temperature.Pay and Benefits*:Pay Range: $ 65,353.60 annually - $ 111,726.16 annually Benefits: Shirley Ryan AbilityLab offers a comprehensive benefits program that is competitive with our industry peers in our geographic locations: and benefits' eligibility can vary by position.
