It focuses on promoting community integration, enhancing self-efficacy, and building a robust network of formal and informal supports. CTI teams conduct assertive outreach and engage patients with multiple behavioral health, medical, and social care needs, such as homelessness or justice involvement, with a particular focus on those who have had frequent contact with the healthcare system. The team supports patients during their transition from the hospital to the community, working to connect them with long-term, stabilizing resources. Ideal candidates for this role will be able to provide flexible patient-centered support in both clinical settings and in the field as needed.Summary Of Duties And ResponsibilitiesThe CTI Nurse will address the physical/medical health needs of program participants in community settings and during their transition to the community after discharge. The nurse will have the unique opportunity to engage with participants who have found themselves in precarious situations that have negative impacts on their physical and mental wellbeing.The nurse will provide interventions that are supportive to or restorative of life and well-being.They will also support connections to traditional medical and specialty care to manage health conditions and prevent escalation and unnecessary hospital admissions. The nurse will also work to reduce health gaps and disparities caused by environmental, physical, and social determinants of health by developing health care plans, providing nursing services, and referring patients with social and emotional needs to other community resources.The CTI team will continue to follow participants during a 9-month period ensuring that they become both linked and engaged with identified community providers, and that their supportFunctional Job DescriptionThis document supplements but does not replace the corporate job descriptionwork, periodic on call coverage, and a willingness to work flexible hours.General Tasks And ResponsibilitiesCore DutiesMaintain an active caseload of patients Conduct health assessments and monitor patients' medical needs. Plan, implement, and evaluate care and treatments in line with evidence-based practices. Provide field-based, flexible support to meet participants' health and social needs during the transition from hospital to community. Participate in periodic on-call coverage and maintain flexible work hours. Prepare accurate and timely reports as needed. Patient Engagement & SupportEngage with patients during inpatient hospital admissions or emergency department visits and participate in transition planning, including needs assessments. Offer ongoing health education to patients on managing health conditions and improving overall well-being. Provide medication reconciliation and educate patients about their prescribed medications, including dosages, effects, and potential side effects. Actively connect patients to community-based resources (e.g., primary care physicians, specialists, rehabilitation). CollaborationDevelop and maintain effective working relationships with healthcare providers in acute and non-acute settings. Collaborate with CTI team members and external agencies to advocate for patients' best interests while ensuring cultural sensitivity and confidentiality.Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).Special NoteFor assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit.
