OverviewMake your application after reading the following skill and qualification requirements for this position.Manage the day-to-day aspects of this program, including managing the program timeline and goals outlined, supervising the Patient Navigators (Peer Counselors), and ensuring accurate record keeping, data tracking, and proper follow-up on all test positives received. Check-in regularly with the Patient Navigators to ensure they are completing their assigned duties and to monitor the linkage to care activities. Work with the data analyst on dissemination and poster/publication submissions.ResponsibilitiesEstablish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors, and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.Contribute to and participate in the Performance/Quality Improvement activities of the assigned department, including data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, and consistent adherence to the specific rules and regulations of the BronxCare Health System's Safety and Security Policies, Risk Management: Incident and Occurrence Reporting, Infection Control Policies and Procedures, and Patient and Customer Service.Coordinate and manage all aspects of the Gilead project including management of the project timeline, project goals, coordinating case management meetings, and liaising with Hospital clinical leadership.Provide project leadership by serving as the "˜brainstorming' engine by continually contributing to ideation methods and work-plan implementation.Coordinate and supervise the maintenance, monitoring, recording of patients' medical records, follow-up, and lab data.Collaborate with various teams on effective adjustments to their management communications, evaluations, planning, and monitoring.Cultivate and present sound recommendations and resolutions to program problems allowing for smooth operation of the multi-disciplinary department management team.With assistance from the Department's Data and Research Analyst, conduct a proper evaluation of the program, including check-ins through the program and a final evaluation at the end, and develop posters/publications for dissemination.Create all policies and procedures for the program and revise as needed.Ensure and account for activities of peer counselors as it relates to the grant goals.QualificationsDemonstrated competence in leadership/program management.Relevant clinical experience.Bachelor's in Allied Field.Master's in Allied Field, MPH.Experience handling sensitive data.Database and computer experience.Extensive computer knowledge.#J-18808-Ljbffr
