WCS Healthcare Partners is seeking a local contract nurse RN Case Management for a local contract nursing job in Bronx, New York. Job Description & Requirements ~ Specialty: Case Management ~ Discipline: RN ~ Duration: 13 weeks ~40 hours per week ~ Shift: 8 hours, days ~ Employment Type: Local Contract Immediate opening for an LMSW Case Manager to work in a large social services agency in the Bronx! The schedule for this position is Monday through Friday, 9am -5pm. Job Function: Assist clients to locate an alternative housing option when a client's health so requires Provide delivery of mental health and alcoholsubstance abuse services and referrals Assesses clients andor family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills. Establishes the social work component of the patientfamily plan of care based on goals mutually acceptable to the client, family and significant others. Makes referrals to other community services, as necessary. Provides psychosocial work services to patient andor family, including short-term individual counseling, community resource planning, and crisis intervention. Job Requirements: ~ Bachelor's Degree in Social Work, Public Administration preferred.
