Title: Community Education Liaison (CEL) Sales Representative Position Summary:Facilitates patient admissions to Hospice by marketing and education activities with contracted and potentially contracted hospitals, skill nursing facilities, respite centers, physicians, and other community resources. Recognizes and performs duties which need to be performed, although not directly assigned. Duties fall within the scope and role as defined by appropriate regulatory agencies and governing bodies.Job Responsibilities:Assists in the development, maintenance and implementation of the marketing and education plan with the Executive Vice President-Administration, in compliance with Hospice policies and all applicable laws and regulationsProvides Hospice in-service programs to hospital and skilled nursing facility staff, physicians, community groups, and others as directed.Markets Hospice services to contracted and potentially contracted hospitals, nursing facilities, area physicians, other agencies, and alternative care programs with potential linkage for hospice careParticipates in staff meetings, staff development activities, and Ascend Hospice meetings, and attends off-site conferences as assigned.Projects Hospice philosophy of care and advocacy for the patients and families.Projects Hospice philosophy and goals in all job-related roles.Participates in performance improvement projects as assigned.Assists in the development, maintenance and administration of up-to-date department policies and procedures which are revised on an annual basisAssists with the development of annual goals and objectives.Maintains awareness of regulatory requirements which impact marketing efforts and goals of the plies with all fiscal parameters and policies.Ensures the accuracy of all public information.Recognizes and performs duties which need to be performed although not directly assigned.Performs responsibilities related to patient admissions according to Ascend Hospice policies and procedures, and in accordance with all applicable laws and regulations.Assists discharge planner and other personnel of contracted hospitals, agencies, and institutions in facilitating patient admissions to Ascend Hospice according to guidelinesInitiates and completes referral forms for Hospice admissions at hospitals, skilled care facilities, and other locations, as appropriate, during site visitsAssesses and counsels prospective patientsfamilies in various settings regarding the services offered by the HospiceCoordinates all designated patients admission activities with the Director of Clinical Services or designeeAssists in the orientation of new personnel and visitors to the hospice program as necessaryRecognizes and performs duties which need to be performed although not directly assigned.The territory will be Cumberland and Gloucester counties, NJ.You will essentially be on the road. The nearest office is in Voorhees, NJ. You can always stop in if you need the office printers, etc.
