Services include assistance with daily activities, Alzheimer'sDementia care, end of life, companionship, and more, tailored to meet individual needs.Role DescriptionThis is a full-time on-site Care Coordination Assistant role located in the San Francisco Bay Area. The Care Coordination Assistant will be responsible for assisting multiple departments with administrative aspects of service delivery and quality assurance, including supporting intakes of new clients, data entry and follow up, calendaring, and more. Additionally, the Assistant will help with coordinating care services and communicating with clients and their families, facilitating company sponsored events and series', and attending community events throughout the San Francisco Bay Area.Assist data entry for service inquiriesMaintain records for partnered Senior Living Communities, as well as submitting paperwork for new partnershipsSupporting start of care and onboarding of new clientsAssisting familiesclients with senior living community toursAssisting in coordination of caregiving services, as neededFollowing up with existing clients to ensure service standards are upheld and satisfaction rates remain highSupporting New Caregiver onboarding with HR team through culture and policy educationQuality assurance visitsProviding direct support to clients in times of emergency as neededCollaborating in the development of new service programs, market research, and promotional opportunitiesCounseling and redirection of field staff as neededAssisting with in-home assessments and walk-through Other administrative duties as assigned QualificationsInterpersonal Skills and Communication skillsPassion for supporting others on and off the teamAbility to work effectively in a team-oriented environmentStrong organizational and time management skillsPrevious experience in a similar role is preferredCare for Seniors is an Equal Opportunity Employer and values diversity at all levels.
