Job Summary: As a Licensed Practical Nurse (LPN) at our healthcare team, you will play a crucial role in delivering comprehensive nursing care to our residents. You will work under the supervision of registered nurses and other healthcare professionals to ensure the well-being and comfort of our residents. Your dedication, clinical expertise, and compassion will directly impact the lives of those in your care. Responsibilities: Perform routine nursing tasks under the guidance of registered nurses, physicians, and other healthcare professionals. Administer medications and treatments as prescribed by the healthcare team. Monitor and assess residents' vital signs and condition, reporting any changes promptly. Assist with resident admissions, transfers, and discharges, ensuring proper documentation and communication. Provide wound care, injections, and other medical procedures as needed. Collaborate with the interdisciplinary team to develop and update individualized care plans for residents. Assist in maintaining a clean and safe environment for residents and staff. Document all nursing care provided accurately and in a timely manner. Educate residents and their families on healthcare procedures and provide emotional support. Respond to emergencies and provide first aid when necessary. Adhere to all state and federal regulations, as well as facility policies and procedures. Participate in ongoing education and training to maintain licensure and stay current with best practices in nursing. Qualifications: Valid Licensed Practical Nurse (LPN) license in state of practice. CPR certification required. Previous experience in a skilled nursing facility or long-term care setting is preferred. Strong communication and interpersonal skills. Ability to work as part of a team and independently as needed. Compassion, patience, and a commitment to providing exceptional care. Basic computer skills for electronic health record documentation.
