Overview The Transitional Care Clinic TCC) Nursing Coordinator maintains accountability for daily operations and delivery of patient care through supervising, leading and mentoring personnel in the TCC. Serves as a professional leader, role model, and resource to department staff and performs personnel actions, staff development and training, evaluation and counseling/coaching. This position will work closely with the physician dyad partner and other system leaders to develop and implement a comprehensive and cohesive model that optimizes coordination of care and care transition across the enterprise and region for high risk patients. This leader will also function as a nurse within the department. Qualifications Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor"˜s Degree, Work Experience: None Responsibilities Provides leadership, vision and management to department. Develops plans to meet strategic objectives and support program growth.Maintains oversight of the day-to-day leadership for department operations including staff development and education, development and monitoring of policies and procedures, protocols, competencies and staff performance. Monitors appropriate record keeping such as payroll processes and monitoring reports.Manages daily nursing and clinical activities and resolves work related issues or assists employees to do so. Makes work assignments and reviews work of direct reports.Performs all job functions of direct reports to include ambulatory nursing, care coordination, home care nursing and telephone triage.Participates in development, planning and monitoring of fiscal budgets. Compensation and Benefits The compensation range for this position is $32.51per hour - $55.92per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
